Admission Information

Preparing for Your Stay

Please be prepared to provide the following for a smooth admission process.

It is necessary to be seen by your physician within five days prior to admission and your physician will update your medical information.

It is also a requirement to have a TB test or chest x-ray completed prior to admission to a care center.

The following documents need to be brought with you on the day of admission:

  • Social Security Card
  • Medicare Card
  • Supplemental Insurance Card
  • Living Will and/or Durable Power of Attorney
  • Power of Attorney Papers
  • Long-term Care Insurance Policy
  • Discharge papers for you or your spouse if you are a Veteran

Please also provide names, addresses and phone numbers of family or friends to contact for an emergency. As well as names, addresses and phone numbers for:

  • Doctor
  • Dentist
  • Eye Doctor
  • Podiatrist
  • Mortician
  • Church or Religious Affiliation

Personal Items:

  • 5-7 days’ worth of clothing (laundry services available; see Laundry Services)
  • Comfortable, rubber-soled shoes (nonskid, good tread footwear)
  • Sweater or Wrap
  • Any assistive devices (wheelchair, walker, cane, etc.)
  • Dentures, Hearing Aids, eyeglasses
  • Hygiene items preferred by resident (hairbrush, toothbrush etc.)

IT IS IMPORTANT THAT EACH ITEM BE MARKED WITH THE RESIDENT’S FULL NAME. AN INVENTORY SHEET WILL BE PROVIDED AND NEEDS TO BE COMPLETED UPON ADMISSION.

Residents/Families/Visitors/Staff

DIA/State Life Safety Codes

No power strips (office use-electronics allowed if proper kind)
No extension cords
No heaters
No curling irons (use allowed if supervised- not left in room)
No hair dryers (use allowed if supervised- not left in room)
No heating pads/electric blankets
No vaporizers/humidifiers/dehumidifiers
No more than 20% of surface coverage with flammable materials- art/quilts/giftwrap/door décor, etc.
No cleaning agents
No flammable aerosols (personal grooming products allowed)
No plug-in air fresheners
No blade-type fans
No trip hazards- cords/tubing in open pathway
No storage on floors- especially cardboard boxes
There must be 18-inch clearance from any sprinkler-head in all directions, including the closet
Nothing hung on sprinklers/pipes or near heat- source/lighting (includes vents/above-bed light)
No bar soap
No throw/scatter/bathroom rugs
No holes placed in doors- ask maintenance to assist
Oxygen tanks must be secured in stand/base
Oxygen sign must be posted on door, if two in the room
Posted sign for staff/notify laundry if WRCC is not providing laundry services
Have staff notify maintenance if want any pictures, clocks, etc. hung on walls
No live Christmas trees/wreaths
No candles/flames
No halogen bulbs
Christmas lights must be U.I. approved, low-heat type

Telephone Installation

Each of West Ridge Care Center’s private rooms is phone ready. Telephone service installation can be completed by calling United Business Tech at 319-298-9056.

Compliance Concerns and Reporting Guidelines

Anonymous reports of compliance concerns or complaints may be reported by sending a written report to the attention of the Compliance Officer or by calling the designated compliance hotline at: 1-855-402-6116

Use the Facility Code: WESTR313

Reports may also be made by calling the Facility and asking for the Compliance Officer. The Facility will publicize how compliance concerns and complaints may be reported.
